Northern Ireland's Historical Abuse Inquiry has begun focusing on the activities of paedophile priest Brendan Smyth. The (HIA) inquiry is examining whether systemic failings allowed Smyth - a member of the Norbertine order - to perpetrate the abuse over more than four decades.
